Tennessee Pharmacists Association (TPA) has partnered with the Tennessee Department of Health (TDH) to serve as the pharmacy profession’s point of contact for information related to the state’s pharmacy-based COVID-19 vaccine response. Complete the following form to sign up, participate, and receive updates from TPA and the Tennessee Department of Health regarding pharmacy-based COVID-19 vaccinations.
To sign up to receive important updates regarding pharmacy’s participation in the state’s COVID-19 vaccination response, CLICK HERE. Please note that all pharmacies interested in providing COVID-19 vaccines will be required to complete the Centers for Disease Control and Prevention (CDC) COVID-19 Vaccination Program Provider Agreement. You can view the CDC agreement HERE. TDH anticipates that CDC will release a new, electronic version of this agreement soon.
Here are the steps you need to complete TODAY, if you haven’t already done so:
- Sign up to participate in the Pharmacy-Based COVID-19 Vaccine Response.
- Review and collect the required information in the CDC COVID-19 Vaccination Program Provider Agreement for future use.
- Complete the electronic CDC COVID-19 Vaccination Program Provider Agreement.
