The Tennessee Pharmacists Association has learned that the State’s Controlled Substance Monitoring Database (CSMD) system is currently experiencing technical issues. TPA has contacted the Tennessee Board of Pharmacy and has received the information below. Please note that the Department of Health has identified a method to allow pharmacists to continue to access the CSMD using the instructions below; however, the Department of Health’s licensure verification website remains inoperable at this time.

Message from the Department of Health:

The CSMD web application is experiencing an issue with verification of board licenses. Please use the “Remind Me Later” button from the popup as it will allow you to enter the CSMD so you can check your patients. We are working diligently to resolve the issue.

When registering the information you provide must match the information as it appears on your Board License. If you experience issues logging into the system (EX: invalid Board License, invalid DEA) please contact the CSMD Admin via email CSMD.Admin@tn.gov or phone 615-253-1305.

The Tennessee Department of Health is committed to protecting the security of the CSMD data and the information of its users. TDH appreciates your patience as we work through this important security measure.

REMEMBER: The first time you login to the system go to MY ACCOUNT in the upper right corner to review and update all necessary information. Review the email address to ensure that it is a private and confidential email that only you can access. Click on the CHANGE PASSWORD and SECURITY QUESTIONS buttons on the left navigation bar and view/modify as needed.

The starting date of this database is 01/01/2010. If you need information from the Archive (Before 1/1/2010), contact CSMD.admin@tn.gov with details of your request.
