      Standard Six is the curriculum you are using for your program. In the attached document, a list of different curriculums are provided. There are other options available. You will only need to make a copy of the cover of what curriculum you are using.

      Standard Five involves the instructional staff and referral mechanism. As mentioned in Standard Four and the attached documents, most of Standard Five is completed. If you plan on having additional instructional staff such as an RN, RD, or RPh, they will need to provide their job description, resume, license, and CE credits. If you plan on using a technician, they will be considered a community health worker. They will need a documented training plan and a policy on supervision.

      Standard Four involves you, the program coordinator. There is a lot of repetition between Standard Four and Standard Five, since you are not only the program coordinator, but the instructional staff. In this standard, you will need your resume (which needs to include your experience with chronic disease), as well as job description for the program coordinator. Also in this section, you will need to include a copy of your CE from the CPE Monitor, as well as a copy of your current pharmacist license.

      Standard Three deals with access to your program. Inside the document, you will find links to resources that will help you identify the population in which you practice. It is important identify and understand your population demographic in order to make sure you are meeting the needs of your patients. Some pharmacy software has this type of information built in, so you can identify the number of patients at your pharmacy with diabetes. It is also important for this standard to identify resources that you have, including room, materials, curriculum, and staffing.

      Standard Two involves external input. Your program needs to have a documented plan for seeking outside input. At a minimum for your advisory committee, you need yourself, a community member with diabetes, and another health professional, preferably a physician. On a yearly basis, you must have a documented meeting with minutes. If you do not have a copy of minutes before your accreditation, you will need to submit one within 6 months.

      Standard One involves the internal structure of your program. While this is a service that most of your pharmacies will provide, it needs to be distinctly different service from the pharmacy itself. Standard One must include a clearly documented organizational structure, a program mission and goals, and a letter of support from your sponsoring organization.
