Forms submitted prior to May 1 annually will be considered for committee service in the next cycle, to be appointed following the TPA Summer Meeting. Submissions received after this date will be kept on file.
NOTE: If you currently serve on a committee, you must submit your interest again to showcase your continued desire to serve for the next year. Current committee members will NOT be automatically reappointed.
